Chronic Care Management

75% of all healthcare dollars are spent on patients that have two or more chronic diseases.

In an initiative to lower the cost of managing this population and provide these patients with better care, Medicare created the Chronic Care Management program. This program pays the practice the patient chooses to coordinate their care $42.80 per month per patient. In order to receive this payment, the healthcare provider is required to spend 20 minutes per month with each patient in coordinating the patient’s care. This time is required to be time the practicespends working with the patient when the patient is not in the office.

Chronic Care Management Systems

In an average primary care practice there are probably 250 active patients that fall into this category. This patient population represents a potential monthly revenue stream of $10,700 ($128,400 annually) and these patients are already under your care.

Provider Requirements are simple for the Medicare Chronic Care Management Program:

  • Patients must be identified as having two (2) or more chronic diseases and be signed up for the program through the provider’s practice.
  • The Provider must set up a system to track the amount of time they spend coordinating the patient’s care.
