75% of all healthcare dollars are spent on patients that have two or more chronic diseases.
In an initiative to lower the cost of managing this population and provide these patients with better care, Medicare created the Chronic Care Management program. This program pays the practice the patient chooses to coordinate their care $42.80 per month per patient. In order to receive this payment, the healthcare provider is required to spend 20 minutes per month with each patient in coordinating the patient’s care. This time is required to be time the practicespends working with the patient when the patient is not in the office.
